I actually just generally stash a few (cloth) diapers in my purse along with a wet bag. (We never get poopy blowouts, so I don't need an extra sleeper or anything.) For a newborn, I'll bring along a receiving blanket for spit-up and I am always wearing a sling or a wrap. I'm a pretty light packer. :)

Sling or MT (regardless of age)
Pacifier (if baby takes one - my DD never did)
diapers (always pack more than you think you'll need - even when they're older just in case you're delayed)
receiving blanket - I use these as changing pads - smaller and wash up nicely.
change of clothes - probably the first five to six months (between spit up or diaper blow outs)
wipes
Hooter hider and burp cloth (it's small enough to go in the diaper bag all the time, even though I only use it selectively - it makes for a good cover up if you forget a blanket!)
For self feeders - reusable plastic monstrosity that suctions to the table at restaurants.
toys
snacks (when appropriate)
crayon roll/coloring book (for the two year old)

So glad I don't have to find room for formula or bottles in there too!
