INVISALIGN TEEN & BLISSTV’s NEW SMILE GIVEAWAY SWEEPSTAKES

Tell us what a new smile would mean for your child for a chance to win a new smile from Invisalign Teen.

OFFICIAL RULES

NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE OR PAYMENT OF ANY KIND WILL NOT INCREASE YOUR CHANCES OF WINNING. Void where prohibited.  All federal, state, and local laws and regulations apply.

Beginning January 17, 2011 and ending January 28, 2011 at 11:59:59 p.m. (ET), entries will be accepted on blissfullydomestic.com.  Mailed entries must be received by January 28, 2011.

Entrants have five ways to enter and can submit up to a maximum of five different entries to the Sweepstakes.  No more than five entries will be accepted per entrant.

WINNER SELECTION: Align Technology, Inc., the makers of Invisalign and Invisalign Teen (“Sponsor”), will randomly select one winner of all eligible entries and announce the winner on January 31, 2010 on blissfullydomestic.com. A random number generator will be used to select winning entry.

PRIZES & APPROXIMATE RETAIL VALUE: The winner will receive free Invisalign Teen treatment for one of their children with an Invisalign Provider to be chosen at Sponsor’s sole discretion. Approximate value is $5,000 and shall not exceed $5,500.

ENTRIES: NO PURCHASE NECESSARY. To enter the Sweepstakes, visit http://blissfullydomestic.com/2011/invisalign-teen-blisstv’s-new-smile-giveaway for details. Entrants can enter in of the following five ways:

1. Write a comment on the Sweepstakes page regarding how a new smile by Invisalign would promote self-confidence in your child.
2. Take the Invisalign Teen Self Assessment with your child, then post a comment on the Sweepstake page summarizing the result of the Self Assessment.
3. Write a blog post that discusses what you think are the best benefits of Invisalign treatment for your teen, then post a link to the blog post in the comments section.
4. Tweet: “I want to win the Invisalign Teen New Smile Giveaway for my son/daughter from @Invisalign and @BlissfullyD (short link to hub page for Sweepstakes)!” Then leave a comment in the comments section linking to their tweet!
5. Send a stamped 3” x 5” postcard with your name, address, and contact phone number to: Invisalign Contests, PO Box 1093, Chino Hills, CA 91709

Each entrant will receive one entry for each of the qualified entries they submit.  Only qualified entries will be accepted.

ELIGIBILITY: The Sweepstakes is open to legal residents of the 50 United States (and D.C.) aged 18 or older, and parent or legal guardian of the child receiving treatment. Employees of the Sponsor, its affiliates, subsidiaries, advertising and promotion agencies, prize suppliers and immediate family members (spouse, parents, siblings and children) and persons living in the same household of each of the above are ineligible. Void where prohibited.

CONDITIONS OF PARTICIPATION: One prize will be awarded. The prize is awarded “as is” with no warranty or guarantee, either express or implied outside of Sponsor’s limited warranty. The Invisalign Teen treatment must be performed on a legal child of the winner. Sponsor reserves the right to substitute prize for an item of equal or greater value in the event an advertised prize is unavailable.

By entering the Sweepstakes, entrants affirm that they have read and accepted these Official Rules. The Sweepstakes shall be governed by and construed in accordance with the laws of the State of California. By entering the Sweepstakes, entrants hereby submit to the jurisdiction and venue of the federal and state courts of California and waive the right to have disputes arising out of the subject matter hereof adjudicated in any other forum. All entries become the property of the Sponsor and will not be acknowledged or returned. Any entry information collected from the Sweepstakes shall be used only in a manner consistent with the consent given by entrants at the time of the entry, with these Official Rules and with the privacy policy, found at http://www.invisalign.com/teen/PrivacyPolicy/Pages/PrivacyPolicy.aspx

Any applicable federal and state tax liabilities are the sole responsibility of the winner and the winner will receive and IRS Form 1099 in connection with any prize valued at over Six Hundred Dollars ($600).

As permitted by law in accordance with the Sponsor’s Privacy Policy located at http://www.invisalign.com/teen/PrivacyPolicy/Pages/PrivacyPolicy.aspx, Sponsor may use information, which all entrants provide about themselves, to send them announcements about the Sponsor’s products and programs via electronic, text and ordinary mail. By submitting an entry, all entrants consent to Sponsor reproducing submitted content on Invisalign web properties and in other marketing activities. An entry, except where prohibited by law, constitutes a grant of permission to the Sponsor or its agents to include the winner’s name on the InvisalignTeen.com site without further notice or without additional compensation.

Entrants agree to be bound by the terms of these Official Rules and by the decisions of Sponsor, which are final and binding on all matters pertaining to this Sweepstakes. The winner may be required to sign and return an Affidavit of Eligibility, an agreement of compliance with federal, state, and local laws, including FTC guidelines, a Liability Release, and where legally permissible, a Publicity Release within 7 days following the date of first attempted notification. Failure to comply with this deadline may result in forfeiture of the prize and selection of an alternate winner. Return of any prize/prize notification as undeliverable may result in disqualification and selection of an alternate winner. In the event that Invisalign Teen treatment is not medically suitable, or treatment is not geographically available to winner, a consolation prize valued at $1,000, may, at the sole discretion of Sponsor, be awarded and an alternate winner may be selected from the pool of entrants.   Acceptance of the prize constitutes permission for Sponsor and its agencies to use winner’s name, photograph, and/or likeness, biographical material and/or information related to entry (including an altered form of the entry) for advertising and promotional purposes without additional compensation, unless prohibited by law. Sponsor is not responsible for any printing, typographical, mechanical or other error in the printing of the offer, administration of the Sweepstakes or in the announcement of the prize. Sponsor will be collecting personal information in accordance with its privacy policy. SPONSOR MAKES NO REPRESENTATIONS OR WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, REGARDING ANY PRIZE OR AN ENTRANT’S PARTICIPATION IN THE SWEEPSTAKES.  BY PARTICIPATING IN THE SWEEPSTAKES, EACH ENTRANT AGREES TO RELEASE AND HOLD SPONSOR AND THE EMPLOYEES , OFFICERS, DIRECTORS, SHAREHOLDERS, AGENTS, REPRESNTATIVES OF SPONSOR, THEIR AFFILIATES, SUBSIDIARIES, ADVERTISING, PROMOTION, AND FULFILLMENT AGENCIES HARMLESS FROM ANY AND ALL LOSSES, DAMAGES, RIGHTS, CLAIMS AND ACTIONS OF ANY KIND IN CONNECTION WITH THE SWEEPSTAKES OR RESULTING FROM ACCEPTANCE, POSSESSION, OR USE OF ANY PRIZE, INCLUDING WITHOUT LIMITATION, PERSONAL INJURY, DEATH, AND PROPERTY DAMAGE, AND CLAIMS BASED ON PUBLICITY RIGHTS, DEFAMATION, OR INVASION OF PRIVACY.

DISPUTES/CHOICE OF LAW: Except where prohibited, each entrant agrees that: (1) any and all disputes, claims and causes of action arising out of or connected with this Sweepstakes or any prize awarded shall be resolved individually, without resort to any form of class action, and exclusively by state or federal courts situated in the state of California applicable to contracts made without reference to applicable choice of law provisions. All actions, proceedings or litigation relating hereto will be insitituted and prosecuted solely within the State of California, Santa Clara County (2) any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, but in no event attorneys’ fees; and (3) no punitive, incidental, special, consequential or other damages, including without limitation lost profits may be awarded (collectively, “Special Damages”), and (4) entrant hereby waives all rights to claim Special Damages and all rights to have such damages multiplied or increased. California law governs the Sweepstakes and all aspects related thereto.
